ROG Destrier warranty claim - warranty form not picking up the serial no?

Pretty much the post title, it looks like either one of the factory fitted bolts wasn't tightened correctly or was cross-threaded, and worked loose. This caused the other bolt to shear off and the chair has now collapsed on one side.

Obviously i want to raise a warranty claim for this, but the warranty form won't accept my serial number. Does anyone know what the best next step is? I've raised a technical query via the contact us page (which did let me select my Destrier chair)

To address your issue with submitting a warranty request, we kindly ask you to provide the following details for further investigation:

  1. On which page did you attempt to submit the RMA request? Is there any error meseege? Or else? 
  2. Could you share the product serial number of your chair?
  3. Please provide a screenshot of the warranty page for your chair.
  4. What is your current country or region?
