No audio on connecting headphones

Hi all, I have been facing this issue since months. Whenever I connects headphone/earphones via headphone jack or Via Bluetooth, It always happen that out of 5/10 times, There will be no sound output through the headphones. For fixing it, I have to restart everytime it happens. Headphone connected icon shows in the status bar. Only the output is not coming. Please give a software update to fix this as soon as possible.

2.  Firmware version your device is on.

Hi, we'd request you follow the below steps:
1. Clear system cache.
2. Try the same under safe mode:
Hold the power button for 5 seconds => When the screen flashes with option => Press & hold power option => You will get an option to restart in safe mode
3. If still you face the issue
