Monitor extended warranty activation

Hi, I wanted to inquire about my warranty status, then saw that my warranty expired within a month, before I had even purchased the product. The VIP member notice that came with the device has no information on how to activate the 36-month extended warranty that was promised inside. 

Its a VG21AQM1A

FW version MCM103.
