What's your phone’s firmware version (Settings > About > Software information > Build number)
Please also change another SD card for a check.
I would suggest you backup SD card files and format it via PC again.
If this problem still appears, please check the card is write protected or attacked by virus
Besides that, you can also try diskpart or SD card formatter tool to format your card.
